Your task is to implement the presentation layer using HTML/ Javascript and PHP scripts.

(Do not use CSS to add styles to the interfaces it would be another task)

You need to decide what and how would be the interfaces, client side validation with javascript.

If anything is not clear you have the freedom to make your own assumptions but you need to document t clearly so that another can look into that and understand. Use comment in the code for easy understanding.
